Self-Pay

Payment options include: cash, check, debit and credit cards. Payment is due at the time of service. Returned checks are subject to a $30.00 fee.

Insurance

Therapy services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is approval required from my primary care physician?

A receipt can be provided for your reimbursement request for using out-of-network insurances.

Appointments & Cancellation Policy

Please Bring the Following to the First Appointment:

  • Patient's insurance card (if applicable)

  • List of current medications (prescribed and over-the counter). Please note dosage and frequency of usage.

  • Information about the patient's medical and mental health history that may be relevant.

In the event that you are unable to keep an appointment, please notify our office at least 24 hours in advance. If such advance notice is not received, you will be financially responsible for the session you missed. Please note that insurance companies do not reimburse for missed sessions.
